Though quickly fixable, I thought this bug was enough to be mentioned. I was playing on a server not hosted by me, and I had to ask the runner of the server to spawn a new Quest Giver after the previous one died. My class was Tinkerer. I had completed all the quests and, somehow, while the now-useless Ghostly Tinkerer was walking around, going in and out of my house, the door he went through completely disappeared. It didn't drop a door, as if it was destroyed, it just vanished. I don't really know too much else about the circumstances, but feel free to ask me questions if necessary.
